Botox is an injectable wrinkle muscle relaxer. It uses botulinum toxin type A, to temporarily relax the muscle. This reduces the appearance of facial wrinkles.

A Botox treatment is minimally invasive. It’s considered a safe, effective treatment most commonly used for aesthetic correction purposes to improve the appearance of facial wrinkles, creases, and fine lines.

Botox was originally FDA-approved in 1989 for the treatment of blepharospasm and other eye muscle problems. In 2002, the FDA approved the use of Botox as a cosmetic treatment for moderate to severe frown lines between the eyebrows.

It was approved by the FDA for the treatment of wrinkles around the corners of the eyes (crow’s feet) in 2013.

The effects are temporary, lasting 3–12 months, depending on the type of treatment.

Botox, or botulinum toxin, is used for mainly 3 purposes:

  • Control of muscle spasm
  • Cosmetic improvement
  • Excessive underarm sweating

Despite botulinum toxin being debatable for its toxicity, Botox is in huge demand and has proven to be a valuable and worthwhile therapeutic protein for cosmetic improvements.

Botulinum toxin is strategically injected into humans in immensely small concentrations and functions by halting signals from the nerve cells reaching muscles, thereby paralyzing them and allowing the muscles to become less stiff.

Medical use of Botox

Beyond cosmetic applications, Botox is also subordinately used in the treatment of a variety of medical conditions, including excess sweating, underarm sweating, migraines, eye squints, and leaky bladders.

Botulinum toxin is presently put to application into the treatment of over 20 different medical conditions, with more applications under fact-finding examination.

  • Chronic migraine
  • Hemifacial spasm.
  • Idiopathic circumvolution cervical dystonia (severe shoulder and neck muscle spasms)
  • Extreme primary axillary hyperhidrosis (severe sweating, sweat glands). Can completely thwart sweat and stink for months
  • Overactive bladder( drippy or draining bladder)
  • Detrusor (bladder wall muscle) overactivity – causing urinary incontinence
  • Post-stroke upper limb spasticity

Seven Facts about BOTOX

Fast facts

  • Botox had and is been the most saleable non-surgical aesthetic treatment, with over 6 million Botox treatments performed each year.
  • Botox is a neurotoxin compound extracted from Clostridium botulinum, an organism present in the natural habitat where it is highly inactive and non-toxic.
  • Botulism, an affliction caused by botulinum toxin, can cause respiratory disruption and prove life-threatening.
  • Botulinum toxin is used to minimize the appearance of wrinkles and fine lines by immobilizing the underlying muscles.
  • Botox is also used to treat muscular disorders, severe sweating, migraines, and some bowel ad bladder issues.
  • Just 1 gram of botulinum toxin could have the capacity to kill over 1 million people. 2 kgs of it could annihilate the whole of the human population on Earth. That being said, is about the potential of Botox, but it is productively executed in the effective medical treatment of certain aging indications and medical conditions and not as a noxious component.
  • It is FDA-approved for the treatment of glabellar lines and in this area specifically. However, it is most commonly used off-label for crow’s feet, horizontal forehead lines, smoker’s lines around the lips, or marionette lines at the corners of the mouth.
  • Botox Cosmetic and injectable dermal fillers are completely different. They function differently under your skin and their results also differ and duration too. Dermal fillers work distinguishably, plumping up tissues so that wrinkles ad lines diminish or disappear.
  • As a general rule, Botox is most often used in the upper portion of the face, and fillers are used mostly in other areas.

What is Botox, and what does it do?

What is Botox?

“Botox is a chemically derived substance from the bacteria called botulism toxin, which is found in many natural settings, including forests lakes, and soil, and has poisonous properties.

The bacterium can also be found in the intestinal tracts of fish and many mammals and the gills and organs of crabs and other shellfish.

But most importantly such naturally occurring specimens of Clostridium botulinum bacteria and spores are usually non-dangerous.

The risk of harm only arises when the spores transfigure into vegetative cells and the cell population keeps increasing exponentially.

At a certain point, the bacteria start generating botulinum toxin, the harmful neurotoxin liable for botulism.

The prime target of Neurotoxins is the nervous system, and they attack by hampering the information-gathering and signaling processes that enable neurons to communicate effectively.

For many, the concept of Botox may sound spine-chilling, but the reality is—when used in very controlled doses, the practice is safe.

The compound is injected into a particular area using a syringe or a blunt needle, and once inside, it binds itself to receptors in the muscle, impacting upon the nerves within.

“So as a possible outcome, when your nerve emancipates a chemical to make that specific muscle precipitate or blaze, it can’t.

The function dwindles, and the fine lines or wrinkle that develops when the muscle contracts will reduce, or vanish completely. It’s not a fixed thing, though. Your body rebuilds those receptors over time.”

Cosmetic treatments

The typical application of BOTOX® for cosmetic treatments is to decrease facial wrinkles and fine lines in older adults, though young adults also have the reasoning to get Botox injections and are also benefiting from it.

The muscles no longer contract after the Botox injection, which is what causes the wrinkles to soften and calm.

The treatments you might opt for BOTOX for cosmetic improvements include:

Diminishing & relieving Lines & Wrinkles non-surgically

Whether it be women or men, it is about human make-up that makes both disposed to lines and wrinkles, be it a reason of aging, smoking, sun damage, or stressing out.

The interesting thing is whether they are those crow’s feet wrinkles, laugh lines, worry forehead lines, or lines between the eyebrows, both get them unbiased and still there is a fuss about inequality of anatomy in both, ironical it is, isn’t it?

That said, you can alleviate the appearance of those lines and wrinkles without having to turn to painfully invasive surgery.

BOTOX® effectively helps with lines and wrinkles on your forehead, neck, and around your eyes and mouth– and it is non-surgical.

Lifting your brow

There comes a phase in everyone’s life when the brows begin to droop and stoop. BOTOX® injections can be used to lift up the lowering brow, which contributes to making your face look less tired, and sad and anything giving a gloomy impression of you, thereby making for an overall youthful look.

Jaw Contouring

Recently, medical researchers have devised a new BOTOX® benefit: Patients with oversized masculine muscles of the jaw (the muscle used for chewing), can be treated with BOTOX® injections.

It works fairly well for people looking to transform their square-shaped faces into oval-face shapes in a non-surgical way.

BOTOX ® Cosmetic injection treatments have been reported to decrease the size of the masseter muscles, causing an amiable curved jaw and cheek.

With Botox, the bone structure does not change; the decrease in the thickness of muscle manifestly refines the angle of the patient’s jaw.

Improving a gummy smile

A gummy smile is a smile where too much of your gums are shown while speaking, eating, or laughing.

As a prototype, it is because of the excessive elevation of the upper lip than is normal.

Upper lip BOTOX® injections weaken the retractor muscles of the upper lip in order to prevent them from rising above your upper teeth anymore when you open your mouth, thus giving a more balanced-looking smile.

Rejuvenating hair

Hair loss, thinning hair, and scanty scalp have become common problems in both men and women, yet there is no single treatment that can prove remarkably productive for all.

After all, it is not something that most people will able to ignore and could result in having detrimental psychological effects on well-being.

BOTOX® is reported to help people with hair loss.

When injected into the scalp, improves blood flow, increases nutrient/protein delivery to hair, and relaxes muscles.

Please note that Botox treatment for hair loss only works when your hair follicles are still “alive” and aren’t completely destroyed.

Botox injection procedure by Dr. Sanchika Gupta in Faridabad

Botox injection preparation

Preparation

How to prepare the Botox Procedure is one of the amazing aspects about Botox because there is no only true preparation required.

There are specific elements that will enhance your likely results and reduce any potential risk or complication of bruising that can occasionally occur from the injection.

The best solution to keep small injection site bruising formed after the procedure at bay is to avoid taking aspirin, or alike products. If you are prone to smoking, your chances of bruising are also increased.

Naturally, before any treatment, an experienced clinician will assess you in order to establish that you are a safe and qualified candidate for the procedure.

You also may have a pre-existing medical condition that causes you muscle weakness, breathing difficulties, swallowing troubles, or speaking challenges.

A certified clinician makes sure that a patient is put to scrutiny to analyse a full history and detailed examination before injecting a treatment such as Botox®.

It will likely be the safe way to achieve an individual’s cosmetic objectives.

During the Botox Procedure

Procedure

If you are willing to receive a Botox treatment for your cosmetic enhancements, you certainly need to visit a clinician’s office. It is an outpatient procedure that is performed only at a doctor’s clinic under certain medical safety standards.

A Botox treatment takes around 15 minutes to complete.

The recipient of Botox injection is placed in a somewhat raised/elevated position on the examination table, and the areas to be injected are first cleansed with a non-alcoholic cleanser, such as Betadine or Hibiclens.

The clinician will then apply a topical anesthetic, such as NumbSkin, Emla, or any good alternative lotion, at this time.

The Botox is then injected into the areas pre-decided for the treatment. Botulinum toxin is prepared for administration by diluting the powder in saline and injecting it straight into neuromuscular tissue.

Characteristic Botox injection patterns include around 4 or 5 areas on each side of the forehead and 2 or 3 areas on either eye region.

However, more areas can also be injected by well-experienced and skillful physicians, typically depending on the kind or intensity of the wrinkles.

Please note that, apply pressure if an area seems to be leaking blood after the injection. While ice can also be applied and is one of the solutions but direct pressure is still much more effective than ice for the management of bleeding and bruising.

The apparent effect of injection would take about 24-72 hours to appear in existence. In rare of rare cases, it may take as long as 5 days for the complete effect of botulinum toxin to be noticeable.

After the Botox Procedure

Post-treatment

After receiving the Botox injection, the patient would generally lay upright or semi-upright on the examination table for about 2-5 minutes to make sure that the substance goes well inside and that the patient feels relaxed after the procedure, and then the patient will be asked to avoid lying down for 2-4 hours.

Painkillers

If bruising happens or is a concern, it will be crucially essential for the patient to avoid taking Anti-inflammatory medications like aspirin or related products, such as Naproxen (Aleve), or ibuprofen (Motrin, Advil), after the procedure to keep bruising to a least.

These medications thin the blood, which can aggravate any bruising post-procedure.

Hence, if you do want to minimize discomfort through painkillers before and after the procedure, paracetamol is the safest option, or choose to take a natural supplement called Arnica Montanna.

Also, make sure to inform your clinician about what medication you are on before having a Botox treatment.

Physical movement in the treated area

Avoid strenuous exercise for the next 24 hours after Botox as it could result in bruising and, in rare possible cases, the Botox could shift to other areas of your face, contributing towards making the muscles in these areas to be temporarily paralyzed.

It is best to also avoid doing yoga for 24 hours after having Botox as inverted positions can also cause Botox to migrate to other muscles in the face.

Exercise

Yes, you can exercise the muscles treated with Botox after 4-6 hours of having the treatment by squinting, frowning, or raising your eyebrows to ensure the treatment works well into your facial muscles.

Consumption of alcohol

Many of you must be eager to enquire about how soon after having Botox could you consume alcohol.

Taking alcohol afterward can increase fluid retention in the body and consequently thin the blood, both of which can escalate any bruising you may feel after Botox treatment.

Some people will find that they experience mild bruising at the needle sites after having Botox injections — if alcohol is present in the system this can exacerbate the bruising.

For this reason, it’s advisable to avoid drinking alcohol for a day or two before and after the procedure.

Apart from possibly raising the risk of bruising immediately after the treatment, consuming alcohol is greatly unlikely to have any long-term harmful effects on the outcomes of your treatment.

If you have a special event coming up where you are likely to be taking alcohol, having Botox at least 7-10 days before is recommended for any bruising to settle down and the full effects of Botox to be seen.

Drive

Can you drive after having a Botox injection?

Yes, of course, you can drive back to your home or wherever as Botox injections will not affect your ability to drive, and neither would have any detrimental effects from the same.

Fly

There have been long-held concerns about flying after taking Botox. The concern revolves around the perception that alterations in air pressure in an airplane cabin can modify the outcomes of Botox.

Changes in air pressure can signalize that Botox can shift to other muscles in the face. So, it is advisable for the patients to avoid flying for 24-48 hours after getting Botox treatment.

Touching/Feeling Face

Yes, you can touch/feel your face after having Botox and you can even continue with a normal skincare routine post-procedure, including face cleansing and moisturizing the skin.

However, you should avoid massaging, scrubbing, or rubbing your face for ideally 24 hours, or at least 6 hours, after getting Botox.

Make-up

You should avoid applying makeup for at least 6 hours after having Botox treatment.

Facials or Microdermabrasion Treatments

It’s best to wait until a week before having facials or microdermabrasion after Botox. You should also avoid laser treatment, chemical peels, and sun beds for at least 10 days after having Botox.

Next steps

Results from Botox injections will be observed within 3 to 10 days.

Images may be clicked earlier in the procedure so that the recipient can verify their outcomes themselves.

It is astonishing to see how many patients do not remember how they looked before the procedure and are surprised at the difference when shown a picture.

Before undergoing the procedure of Botox, the patient should understand that Botox does not really erase lines but calms them.

What this means is that the deeper lines will become rather relatively less deep and superficial lines will almost disappear.

This can be somewhat related to the act of steaming an apparel’s shrinkages rather than ironing them.

It may be a good option for the patient to have additional procedures, such as dermal filler products such as — Radiesse, Restylane, Perlane, Sculptra, or Juvederm in order to flesh up the wrinkles or lines that are now calmed.

Furthermore, it may be required to receive two or three sessions of Botox treatment for deeper lines before the procedure become effectual and results appear optimal.

For Botox to be used in conjunction with dermal fillers, the area of the crease between the eyes is a specifically perfect area for treatment as these obstinate wrinkles don’t always respond wholly to Botox alone.

There has been some disagreement of thoughts about using dermal filler in combination with Botox in that area as it may inhibit arteries or veins and cause blood loss and scarring to the area.

Generally, the smaller particle fillers like Juvederm Ultra or Restylane, are best suited in the area between the eyes for this reasoning.

What happens when the results of Botox die?

Fading of results

The results of Botox will remain up to 10-12 weeks (about 3 months) in most patients. After about 3 to 4 months, the patient will slowly start to notice that the areas that have been treated by Botox will be returning to their pre-Botox look.

Having Botox literally does not augment or worsen wrinkles so if you feel your wrinkles look more identifiable after the Botox treatment has weakened or vanished, it will most likely be because you are developing more wrinkles with age or have become used to your face looking plane & unwrinkled.

Risks of Botox Injections

Side effects

Risks are very minimal with the procedure of Botox. The main risks associated with Botox treatment are pain, headache, and flu-like sickness.

It is significant for the cosmetic specialist to evaluate the patient’s lids before injecting since the individual may not be a good candidate if he or she has an excessively droopy lid to start with or one that is held up by always arching the lids.

Ptosis is a condition of a severe drooping of the eyelid that can happen in up to 5% of patients but is quite atypical if the clinician is skillful and performs the procedure often.

These side effects are typically very minor occurrences and resolve with time.

Lately, there have been concerns about retrograde Botox transmission, meaning that the substance could move back to the central nervous system, causing long-term damage.

A medical research study by Flavia Antonucci in Italy has been conducted chiefly on the crude form of the toxin and not any of the market-available preparations.

The study has been undertaken on animals and with the injection of the toxin to one area and in a concentration of approximately 150 times greater than normal injections for cosmetic indications, which are distributed over multiple sites in humans.

Concern is particularly objected at a specific population of patients getting more than the usual doses of botulinum toxin and not targeted at the normal user of Botox, per se.

There are some yet rare, but prospective side effects, that you should be aware of before having Botox. These include:

  • Slight Swelling (similar to midge bites)
  • Bruising/Redness at the injection site
  • Infection
  • Allergic reactions such as rash, pruritus
  • Anxiety
  • Nausea and/or headache
  • Flu-like symptoms— periodic numbness
  • Drooping of the eyebrow or upper eyelid
  • Visual disturbances— blepharitis; photosensitivity
  • Dry Skin, dry mouth, and dizziness
  • Occasional sensation of ‘needles’ and ‘pins’
  • Antibody formation (significant deterioration in response)

Botox results

The results generally begin to show up to be visible within 3 to 10 days or even sooner. Typical results are wrinkle-free, contoured, smoother, younger-looking skin; which lasts about three to 4 months.

Over time, the activity of muscles as well as other areas will steadily resume to its usual, depending on the amount injected.

The praiseworthy thing about Botox cosmetics is that it tends to work all right even up to the third month, as a cosmetic treatment that might last a short span at full vigor and then wear off quickly.

Results largely vary depending on who is giving the injection to the patient.

It is of utmost importance to go to a cosmetic surgeon or skin specialist like Dr. Sanchika Gupta who is dexterous at this procedure and does it (rather than having an assistant, nurse or other non-physician do it).

The manufacturers too insist that Botox medication be injected by the physicians themselves.

As with most procedures, with Botox too, patients rely upon the proficiency skill of the practitioner.

In addition, there is rare or less likeliness to end up with botched results if the procedure is carried out by a qualified & experienced practitioner.

When to seek medical care for Botox compilations?

If you have got your eyelids stooping or wilting after a Botox procedure, it is recommended to let the physician know because there is a medicine help available to cure this condition.

Any other concerns post procedures, such as rashes, redness, or difficulty breathing, should be reported to the physician immediately.

Bruises occurring after the procedure usually settle within one to two weeks.

However, there is medical assistance available for the treatment of these, such as Procedural Bruise Relief, vitamin K serums, Vitamin K topical treatments, Glymed, etc.

Who are not suitable for Botox treatment?

Best candidate

Botox Cosmetic is FDA-approved for people aged 18 to 65. But you shouldn’t still have it for the following reasons:

  • People who have had an allergic reaction to the drug or any of its ingredients in the past.
  • Or are allergic to a different botulinum toxin brand (such as Dysport, Xeomin, or Myobloc,) or previously had any side effects from these products.
  • If the recipient is allergic to eggs, it is very crucial to not use this substance because it is prepared with an albumin (egg) base.
  • Have a skin issue, infection, or other concern in the injection area.
  • Suffering from disease called myasthenia gravis, Lambert-Eaton, or ALS (Lou Gehrig’s disease) syndrome or another ailment that affects your nerves or muscles.
  • Have Asthma or difficulty breathing
  • Having bleeding problems
  • Some issues with swallowing
  • Plan to have surgery or already have had facial surgery in the past
  • Have weakness in forehead muscles.
  • Suffer from excessively droopy eyelids
  • Are constantly consuming or have recently taken certain medications, supplements, or vitamins.
  • Botulinum toxin should not be administered in pregnant or lactating women.

Botox Cosmetic is not supposed to move far enough through the body to impact a fetus or breastfeeding infant. However, for moral virtue, medical studies have not been conducted on expectant or new mothers, so nobody knows, for sure.

Off-Label Uses of Botox

Botulinum toxin is also used off-label (not approved) for:

  • Gummy smile
  • Correction in Chin, Neck, and other areas of the body
  • Cerebral Palsy
  • Anal fissure and anismus (dysfunction of the anal sphincter).
  • Allergic rhinitis (hay fever)
  • Achalasia (a throat problem that makes swallowing troublesome)
  • Sialorrhea (producing too much saliva)
  • Sphincter of Oddi (hepatopancreatic) palsy- causing abdominal pain)
  • Oromandibular dystonia (compelling diminution of the face, jaw, and/or tongue)
  • Laryngeal dystonia (forceful contraction of the vocal cords).

Many of these conditions are under scrutiny at this time for approval by the FDA.

Moreover, topical forms of botulinum toxin (Revance) are also currently under investigation.

With time, these will likely show up in the market and be soaked into the body for treatments of which Botox Cosmetic is used.

Botulinum toxin is sold commercially under the names:

  • Botox, Botox cosmetic, Vistabel (botulinum toxin type A or OnabotulinumtoxinA)
  • Dysport (botulinum toxin type A or AbobotulinumtoxinA)
  • Bocouture, Xeomin (botulinum toxin type A or IncobotulinumtoxinA)
  • Myobloc (botulinum toxin type B or RimabotulinumtoxinB).

It is important to know that these products are similar but not the same product as Botox. Additionally, these have different units of measurement in correlation to Botox.
